2. Describe the Issue Clearly Provide a clear summary of the issue. If applicable, include examples of error messages or unexpected behavior.
3. Session or Patient Context If the issue relates to a specific session or patient, include the Patient Name, Session Date & Time, Activity Name, Device Used, and Location.
4. Steps to Reproduce the Issue Describe the steps you took leading up to the issue, including the page or section of the app, actions performed, and the resulting behavior.
5. Attachments (Optional but Helpful) Attach any relevant screenshots, recordings, exported data, or error logs that can help us understand the issue better.
